重症大動脈弁狭窄症を合併した大腿骨近位部骨折患者における治療成績の検討

目的：大動脈弁狭窄症(AS)は周術期危険因子であり，高度AS患者の麻酔・手術はリスクが高く治療方針に難渋する．ASを合併した大腿骨近位部骨折患者における周術期リスクについて検討した．対象と方法：2017年1月〜2021年12月に手術を行った大腿骨近位部骨折症例462例を重症AS(S群)，軽症〜中等症AS(M群)，非AS(N群)に分け，周術期合併症，30日死亡率，入院中死亡率，1年死亡率を評価した．結果：周術期合併症，死亡率，1年生存率は各群で有意差を認めなかった．まとめ：重症AS患者であっても厳重なモニタリング下に可及的早期に手術加療を行うことが可能であると考える．
