外反母趾に対する保存療法は一般的には最初に選択されることが多い．具体的には靴指導，運動療法，装具療法，薬物療法などが行われている．「外反母趾診療ガイドライン2022(改訂第3版)」では運動療法と装具療法の有用性についてCQを設定し，検討した．軽度から中等度の外反母趾に対して，運動療法や装具療法単独での変形矯正効果や除痛効果を認める質の高いエビデンスは十分ではなく，推奨文として「軽度から中等度の外反母趾に対して運動療法を行うことを弱く推奨する」「軽度から中等度の外反母趾に対して装具療法を行うことを弱く推奨する」とした．
