脳原性麻痺における癒着性尖足に対する筋膜ハイドロリリース

難治性尖足に対して，液体注入により水力学的に腓腹筋/ヒラメ筋膜間を剥離するハイドロリリースを行い改善することを発見した．対象と方法：脳原性麻痺に合併した尖足5例6足(平均年齢34.4歳)に本法を行った．エコーガイド下に腓腹筋/ヒラメ筋筋膜間へ局所麻酔薬30〜60mLを注入した．結果：本法施行直後，全例に足関節背屈可動域の改善を認めた．獲得角度は膝関節伸展位足関節背屈角度(DKE)平均17.7°，膝関節屈曲位足関節背屈角度(DKF)平均12.8°であった．まとめ：本法により短期的に改善がみられる癒着性尖足拘縮という病態が証明され，新たな治療法となると考えられた．
