日本では境界型形成不全患者が多く，股関節の安定化には軟部組織による安定性が重要と考えられる．われわれは，股関節の安定性に重要とされる腸骨大腿靱帯(IFL)を可能な限り温存するためにポータル間切開は用いずに，前外側ポータルを15〜25mm程度に延長し，関節唇やCAM病変を適切に処置できるようにして手術を行っており，腸骨大腿靱帯の大部分が温存可能である．股関節周囲の解剖を確認しつつ，われわれの術式を紹介する．
