脊椎内視鏡手術の歴史と発展

わが国における脊椎内視鏡手術の開発の歴史と技術認定制度，インシデントレポートシステムについて述べた．わが国では1970年代に経皮的な脊椎手術は開発されていたが，内視鏡は用いられていなかった．術野を視認できる脊椎内視鏡手術の嚆矢は腰椎に対する前方アプローチであった．その後，後方からのヘルニア摘出術から始まり，徐々に適応が頚椎を含めた脊柱管狭窄の椎弓切除(椎弓形成)へと発展していった．この間に日本整形外科学会が脊椎内視鏡下手術・技術認定制度を他科に先駆けて発足させた．これはのちにこの手術手技で診療報酬の請求が可能となったことにつながる．現在では経皮的なアプローチがさらに進化し，その低侵襲性から広く行われることとなっている．さらに手術器具の工夫によりfull endoscopeでの固定手術も可能となってきている．今後，外科手術に関連するAIが進歩すると，術前のより正確な病態把握が可能となり，さらに侵襲の少ない手術が行われる可能性が出てきた．手術手技の習熟，手術の効率性，安全性も格段に進歩することが期待される．
