椎間板性腰痛の痛みを視る

文献概要

High signal intensity zone(HIZ)はT2強調MRIで椎間板線維輪内に認められる高輝度領域であり，椎間板性腰痛との関連が報告されている．当科ではHIZを伴う腰痛患者に対し，椎間板造影ブロックを行い椎間板性腰痛と診断した場合，局所麻酔下のfull-endoscopic discectomy and thermal annuloplasty(FED-TA)を行っている．これまで数多くの椎間板性腰痛患者を経験し，椎間板内の術中鏡視所見を通して，HIZを伴う椎間板性腰痛患者の椎間板線維輪には炎症による発赤，新生血管や出血がみられることがわかった．
