難治性慢性腰痛の一般的なリゾトミー根治術には,高周波焼灼術と低侵襲の内視鏡リゾトミーの2種類がある.神経線維は,手術器具で切断したり,化学物質や電流で焼いたりすることで処理される.最近のBASGに代表されるMRIの細径神経の可視化はめざましい.併せて内視鏡などの光学機器の進歩には目を見張るものがある.後枝内側枝を確認して確実に処理を行う選択的リゾトミーをはじめ今後さまざまな分野に内視鏡の技術が応用されるであろう.
