硬膜異常を伴う脳表ヘモジデリン沈着症

はじめに

　脳表ヘモジデリン沈着症(superficial siderosis：SS)は，くも膜下腔での持続的あるいは反復的な出血によりヘモジデリンが脳，脊髄表面に沈着して，酸化ストレスにより神経障害を引き起こす疾患である．特に脳幹部や小脳に沈着を起こしやすく，小脳失調や感音性難聴，錐体路障害などの中枢神経症状を進行性に引き起こす．40歳以降の男性に好発する．原因としては，脊髄腫瘍や動静脈奇形などの血管異常，頭部外傷でも起こり得るが，髄膜異常に起因するものがあり，近年注目されている．過去の報告では，SS患者48例中40例(83％)に頭蓋または脊髄硬膜異常を認め，48例中21例(44％)に硬膜欠損を認めた．その中でも，硬膜腹側欠損が9例であったと報告されている¹⁾．近年，このSSという疾患の認識が脳神経内科，整形外科，脳神経外科，放射線科の領域で広がっており，比較的早期に発見されるケースが増えてきている．当科では硬膜異常を伴うSS患者を多数治療しており，これまで60例近い症例を経験している．本稿では，硬膜腹側の「硬膜欠損」に伴うSSに関してその特徴，診断，治療について述べる．
