胸郭出口症候群のメディカルリハビリテーションを行ううえで，解剖と病態の把握は重要である．重要なポイントは，斜角筋間隙，肋鎖間隙，小胸筋下間隙での神経血管束の圧迫・牽引をいかに減ずることができるかということである．そのためには肩甲胸郭関節の機能が重要な鍵を握る．また，尺骨神経をはじめとする末梢神経の滑走不全・伸張性低下が症状に関与していることも多い．本稿では，胸郭出口症候群の保存療法について，肩甲胸郭機能不全と神経滑走性・伸張性に焦点を絞って概説する．
