足根骨癒合症(距踵関節癒合症)の診断と治療

足根骨癒合症は足根骨の先天性分化不全が原因とされ，発生率は1〜13％で，50％が両側性である．その中でも距踵関節癒合症の頻度が最も高い．症状は内果後下方に骨性の突出［二重内果(double medial malleolus)］を認め，癒合部に運動時痛や軽微な捻挫を契機とした長引く疼痛を認める．またrigidな扁平足の原因でもあり，距踵関節の可動域制限を認める．診断は単純X線写真では困難な症例も多く，癒合部の診断，評価にはCTが有用である．治療は保存治療に抵抗性の場合，癒合部切除術が有効である．
