乾癬性関節炎(PsA)の臨床症状と診断

乾癬性関節炎(psoriatic arthritis：PsA)は皮膚や末梢関節のみならず，脊椎にも影響を及ぼす炎症性疾患である．PsAは乾癬患者の10〜30％で報告され，多くの症例で乾癬が筋骨格系の症状に先行して発症している．PsAでは，一般的には付着部炎，爪病変，指趾炎などの関節外の症状を併発し，さらには心血管疾患，代謝疾患，精神疾患などの併存疾患を伴うこともある．乾癬の皮膚症状のみならず，長期にわたる炎症による運動器の構造的損傷や機能障害，そしてそれに伴う社会経済的影響を防ぐために，PsAの早期の診断と治療が極めて重要である．本疾患における治療選択肢の増加，特に疾患修飾性抗リウマチ薬(生物学的製剤や標的合成薬を含む)の登場は，皮膚および関節病変の治療に劇的な改善をもたらしている．本稿ではPsAの臨床症状と診断について概説する．
