TKA Alignment,概念の変遷と用語の定義について

人工膝関節置換術が一般的となって約半世紀,いまかつてない大きなパラダイムシフトを迎えようとしている.これまでgold standardと考えられていた,機械的安定性を重視するmechanical alignment(MA)ばかりでなく,personalized alignment(PA)という患者個別に最適ゴールを目指す新しい方法が芽生えてきている.しかしPAには多種多様の方法が提唱されており,その目的とするところも微妙に異なる.その結果混乱がみられていることも事実である.各アライメント手法の目指すところを,正しい用語とともに使用し,おのおのの方法を理解したうえで建設的な議論が必要である.
