腰痛のRed Flags

腰痛のred flags(RFs)を文献検索すると，1994年に米国AHCPR(Agency for Health Care Policy and Research：医療政策研究機構)が策定した急性腰痛診療ガイドライン(以下，GL)において，その記載を初めてみることができる1)．このGLには，“potentially serious spinal condition-tumor, infection, spinal fracture, or a major neurologic compromise, such as cauda equina syndrome, suggested by a red flag”とある．すなわち腰痛のred flagとは重篤な脊椎疾患(悪性腫瘍，感染，骨折，および急激に悪化する神経症状，馬尾症候群を有する脊椎疾患)，これらを疑うべき一連の臨床症状および所見であると言える．本稿では，腰痛とred flagsについて，わが国の腰痛診療GLを紐解きながら解説する．
