それ本当に“いわゆる五十肩”ですか？—注意すべき鑑別疾患

はじめに

　“いわゆる五十肩”とは，特に誘因なく肩の疼痛と可動域制限を生じる疾患である．肩関節疾患のなかでは遭遇する機会が多いが，「放っておいても治る」と日常診療で軽視されがちである．しかし，初期診断において腱板断裂やインピンジメント症候群などとの鑑別ができていない症例をしばしば経験する．また“いわゆる五十肩”に類似した症状を呈する稀な疾患も存在する．

　本稿では，初診時に必ず撮影される肩関節X線検査で診断可能である変形性肩関節症，肩石灰性腱炎，転移性骨腫瘍，頚椎疾患である頚椎症性神経根症や頚椎症性筋萎縮症は省いて，“いわゆる五十肩”の鑑別のためのコツとピットフォールを概説する．
