脛骨人工骨幹置換術を施行した多発性骨髄腫の1例

脛骨骨幹部腫瘍切除後の再建にはさまざまな方法があり，おのおのに利点・欠点がある．多発性骨髄腫の脛骨病変に対し人工骨幹置換術を施行した症例を報告する．76歳女性，X線で脛骨に骨溶解を認めたため近医より紹介され，切開生検で多発性骨髄腫と診断した．切迫骨折であり化学療法前に腫瘍切除および人工骨幹置換術を行った．術後1週で杖歩行が可能になった．術後2年を経過するが疼痛はなく歩行に支障はない．骨転移や全身性腫瘍疾患では，performance statusの改善が予後に影響することがあり，早期歩行が可能である人工骨幹置換は有用な選択肢になり得る．
