第1肋骨疲労骨折に対する体外衝撃波治療の治療経験

第1肋骨疲労骨折は上肢を酷使するスポーツで発生し，保存療法では長期のスポーツ制限が必要となるケースがある．体外衝撃波治療(ESWT)は本邦において難治性の足底筋膜炎にのみ保険適用とされているが，国際体外衝撃波学会の適応基準において偽関節，筋腱付着部障害，疲労骨折は標準的適応となっている．われわれは第1肋骨疲労骨折6例にESWTを施行し，良好な結果を得た．ESWTの試行回数は平均4.3回で，疼痛の改善を認め，全例がスポーツに復帰した．現時点では合併症の出現なく経過しているが，今後も長期的なフォローアップの必要性がある．
