Ⅰ．はじめに

　頸部腫脹を主訴に耳鼻咽喉科を受診する患者は少なくない。多くは口腔咽頭などの炎症に続発する頸部リンパ節炎などの炎症性疾患であるが，悪性リンパ腫や頭頸部悪性腫瘍の頸部リンパ節転移による初発症状として認められることもある。さらに，検査により悪性腫瘍の転移が疑われ，全身的な原発巣検索を十分に行ったにもかかわらず原発巣が不明の転移性頸部癌もある。リンパ節腫脹の場合，炎症性のものか，転移リンパ節かを考慮することは，癌治療の第一歩ともいえるものである。

　本稿では，癌の転移によるリンパ節腫脹を疑った場合の原発臓器精査と悪性リンパ腫に焦点を当てて述べる。
