Ⅰ　はじめに

　嚥下障害を起こす原因疾患，あるいは基礎疾患は，呼吸器疾患，神経疾患，腫瘍性疾患などきわめて多岐にわたる。また，加齢による機能低下が嚥下障害の発症を促進する。嚥下障害を訴える患者を前にして，障害の程度をどのように把握するか，正常嚥下機構のどの部分がどのように障害されているのか，どのように治療を進めていくべきか，原因疾患，あるいは基礎疾患（多くは他科領域でなじみがない）の治療とどのように調整するか，などに関して一定の方式は確立されていない。病態によって，あるいは基礎疾患によっては適切な治療法をみいだせないことも稀ではない。嚥下を「摂食を含めた一連の動作」と考えると，認知期，捕食・咀嚼期，口腔期，咽頭期，食道期に分けられる。なかでも，口腔期と咽頭期の過程が重要である。嚥下に関与する器官を担当すること，嚥下障害の手術治療を行えることから，耳鼻咽喉科医が嚥下障害を訴える患者の診療に果たす役割はきわめて大きいといえる。

　本稿では，まず正常嚥下機構を簡単に説明し，続いてその破綻をきたした状態（嚥下障害，誤嚥）と診断・治療にどのように取り組むべきかについて述べる。
