好酸球性鼻・副鼻腔炎症におけるプロスタグランジンD2/E2代謝の位置付けと治療への展望

Ⅰ はじめに

 プロスタグランジン(以下,PG)は脂質メディエーターの1つであり,免疫・アレルギーを制御する分子として重要である1)。なかでもPGD2およびPGE2は気道炎症と密接に関与する2)。PGはそれぞれに特異的な受容体に結合し生理作用を発揮する。受容体遺伝子のクローニングが進み,現在までにPGD2には2種類の,またPGE2には4種類の受容体が同定されている。さらに共役する細胞内情報伝達機構が明らかとなり,改ためてPGD2およびPGE2の多彩な作用が注目されている。

 本稿では,鼻・副鼻腔におけるPGD2/PGE2代謝関連分子(PG自身や合成酵素あるいは受容体発現など)の発現について,われわれの研究結果を交えながら最近の知見を紹介し,アレルギー性鼻炎やいわゆる好酸球性副鼻腔炎などの好酸球性鼻・副鼻腔炎症におけるPGD2/PGE2代謝の臨床的な意義について概説する。

 また,鼻・副鼻腔における好酸球性炎症には,好酸球自身はもとよりマクロファージやリンパ球などの浸潤細胞,および線維芽細胞や血管内皮細胞などの構築細胞が,細胞接触あるいは産生するケモカイン・サイトカインなどを介して関与すると考えられる。今回は好酸球性炎症の制御細胞としてこれらの細胞の中からT細胞を取り上げ,PGD2/PGE2によるT細胞機能の制御について諸家の報告に自験例を交え概説し,鼻・副鼻腔の好酸球性炎症に対するPGD2/PGE2代謝を利用した制御戦略の可能性について考察する。
