Ⅰ．はじめに

　頭頸部悪性腫瘍は，発声・嚥下・咀嚼・呼吸・審美にかかわる部位に発生するため外科的治療ではそれらの機能が犠牲になることも多い。それに対して放射線治療は形態機能温存的な治療であり，頭頸部悪性腫瘍の治療においては，根治的放射線治療とともに手術と組み合わせた術前・術後照射なども施行され，非常に大きな役割を果たしている。しかし，頭頸部放射線治療も特有の有害事象をきたし，その発生は患者のQOLを低下させ，重篤な場合には機能喪失や致死的になることさえある。

　頭頸部悪性腫瘍は元来高齢者に多くみられる疾患であり，放射線治療の抗腫瘍効果自体に高齢者特有の傾向がみられるわけではない。しかし，多くの臨床試験に登録される高齢者の割合はその疾患全体に占める高齢者の割合より少なく^1），臨床試験の結果がそのまま高齢者の診療に適応できないことは往々にして経験されるところである。高齢者では糖尿病や循環器疾患などの合併症の頻度が高く，それらが治療法の選択や予後を大きく左右する^2）。さらに，放射線治療の有害事象の発生は高齢者ではより著明なADLの低下をきたし，治療の中断や治療の長期化により局所制御に悪影響を及ぼす。そのため高齢者では特に有害事象の発生を最少にするような細心の放射線治療が要求される。
