1．急性扁桃炎に対する抗菌薬治療

Ⅰ．はじめに

　急性咽頭・扁桃炎は，日常臨床において最も頻回に遭遇する上気道感染症である。原因は，ウイルス感染あるいは細菌感染とされるが，一次的に細菌感染で発症する例は少なく，ほとんどはウイルス感染に続発する。細菌感染としては，A群β溶血性連鎖球菌が最も重要視されている。急性咽頭・扁桃炎の臨床像は，咽頭粘膜の発赤のみを認める軽症から，強度の粘膜腫脹とリンパ節腫脹を認める重症例までさまざまであるが，臨床経過は比較的良好であり，多くは抗菌薬と非ステロイド性鎮痛薬の投与により数日～1週間程度で治癒する。そのため，急性咽頭・扁桃炎に対する治療選択においては，(1)A群β溶血性連鎖球菌であるかウイルス性であるかを判断し，(2)その重症度に応じた治療選択を行うことが重要となる。

　本稿では，急性扁桃炎の起炎菌，臨床重症度分類について述べるとともに，急性咽頭・扁桃炎に対する抗菌薬治療について解説する。
