小児中耳炎からみた抗菌薬の適正使用と課題

Ⅰ　はじめに

　20年ほど前には，小児急性中耳炎はどんな抗菌薬を処方しても治癒する疾患であった。当然のことながら，この疾患に対する学問的な関心も低く，特別な合併症を引き起こさない限り学会発表に取り上げられることも稀であった。しかし，1990年代の半ば以降，入院を要するような難治性の急性中耳炎患者が増加し，それとともに学会においても重要なテーマの1つとして取り上げられる機会が増加した。この傾向は現在に至るまで続いている。

　小児急性中耳炎の難治化にかかわる最も大きな因子は，中耳炎起炎菌の薬剤耐性化とこの耐性菌の急速な拡散である^1,2）。中耳はその解剖学的特徴により抗菌薬を内服しても十分な組織内濃度が得られにくい。これが，他の市中感染症に先駆けて耐性菌の蔓延が疾患の難治化につながった原因と考えられる。3歳までに70％の子どもが罹患するといわれる急性中耳炎を通して，耐性菌時代の抗菌薬の適正使用と課題に関して考えてみたい。
