病態に対応しためまいの治療―特に難治症例への対応を中心に

Ⅰ　はじめに

　めまい症例では，同一の疾患であっても病期，重症度，治療に対する反応などの病態は大きく異なっている。めまいの治療に当たっては，その病態に合わせるのは当然であり，そのためには，治療の選択肢をできるだけ広く確保する必要がある。本稿ではこのような観点から，耳鼻咽喉科で遭遇する急性期めまい，代表的な末しょう性前庭疾患である良性発作性頭位めまい症とメニエール病，治療に困難な事例が多い慢性・持続性めまい症例について，その病態に応じた治療，特に難治症例への対応を中心に概説する。
