喉頭癌治療後の音声に関するQOLの検討

Ⅰ　はじめに

　喉頭癌に対する治療法は単独あるいは化学療法併用での放射線治療，喉頭微細手術，喉頭部分切除，喉頭亜全摘術，喉頭全摘術など多様化している。例えば，声門癌T1症例に対する治療成績では，放射線治療と内視鏡下レーザー手術の間に大きな差はない^1～3）。進行癌に対しては，喉頭全摘＋術後照射と化学療法併用放射線治療（＋必要に応じて救済手術）の治療成績に有意差はないとされる^4～6）。したがって，治療後の有害事象，後遺障害は治療法選択に大きな影響をもつ。治療後の機能とQOLを測定して後遺障害を評価することは，新たな治療法の有効性の検討にも重要である。しかしながら，喉頭癌患者の治療後音声に関するQOLをいかなる治療を受けた患者も同一の基準で比較検討した報告は少ない。本研究の目的は各治療法による音声に関するQOLを同一評価方法を用いて検証することである。
