4．滲出性中耳炎

Ⅰ．はじめに

　頭頸部癌診療において，放射線治療は重要な地位を占め，早期癌のみならず，進行癌においても効果的な治療とされている。部位としては特に上咽頭癌^1）に有効性が高く，切除不能癌^2）に対する治療の主体として，あるいは喉頭温存^3）や，術後補助療法^4）を目的に行われることが多い。

　しかし，放射線療法の有害事象として，照射野に側頭骨が含まれる場合に外耳炎・中耳炎・感音難聴，側頭骨壊死などを呈する症例もしばしば経験し，その1つとして滲出性中耳炎（OME）が知られている。放射性OMEの代表的な写真を図1，2に示す。

　本稿では，放射性聴器障害の1つであるOMEについて，上咽頭癌放射線治療例における発生頻度，臨床経過について述べる。
