片頭痛性めまい―その病態の解明にむけて

Ⅰ　はじめに

　『めまい』はさまざまな疾患において生じる症状であり，その病巣・病態とも多岐にわたる。メニエール病や良性発作性頭位めまい症のように病態の解明が比較的進んでいる疾患も少なくないが，一方で，その頻度が高いにもかかわらず，依然，病巣や病態が不明である疾患もある。『片頭痛性めまい（migraineous vertigo：MV）』もその1つではなかろうか。わが国の耳鼻咽喉科医のなかでは，そうした疾患概念があること自体あまり認知されていないというのが実情であり，最近の文献を除くと，報告は少数にとどまる^1）。潜在的には多くの症例が存在すると考えられる『片頭痛性めまい』について，われわれの行った臨床研究を含め，これまでの知見について紹介し，その病巣や病態について考察する。
