Ⅰ．はじめに

　唾石などの炎症疾患に対する顎下腺摘出術は耳鼻咽喉科専門研修において比較的早期に取り扱われる手術である。しかし，顎下腺周囲には顔面神経下顎縁枝や舌神経，舌下神経などの神経があり，その損傷のため麻痺を生ずる危険性がある。また，周囲を走行する顔面動静脈の処理が不十分であると，術後出血のために頸部，口腔底，さらには喉頭の血腫を生じ，呼吸困難のため気管切開が必要なこともある。これらの合併症を回避し，確実でスピーディな手術を行うためには顎下部の解剖を熟知しておく必要がある。以下に実際の手術の手順に沿って顎下腺周囲の臨床解剖と注意点について解説する。
