Ⅰ．はじめに

　急速な高齢化社会を迎え，高齢者の急性感音難聴を治療する機会が増えてきている。一般に高齢者は身体機能が低下しており予備能や再生力も弱いため，循環器や呼吸器などの慢性疾患や精神神経疾患を罹患していることが多い。そのため，①すでに多種類の薬剤投与を受けている，②生理機能低下のため薬剤の代謝・排泄が悪くover-doseとなりやすい，③免疫力・再生力低下のため薬効が得にくい，④服薬指導の内容を理解していない，などさまざまな問題に遭遇し，治療に難渋するケースが多々ある。高齢者の急性感音難聴の診療に当たっては，各疾患における加齢の影響や診断上の特徴，治療の要点を耳鼻咽喉科専門医として理解するだけでなく，老年医学領域を含めた医学全般の総合的な知識が必要となる。

　本稿は高齢者に起こりやすい急性感音難聴の種類，その治療上の問題点，高齢患者のバリアントについてその概要を述べる。
