Ⅰ　睡眠時無呼吸症候群の概要

　1976年にGuilleminaultら^1）は，習慣性のいびき，日中の強い眠気，起床時の頭痛などの症状を有し，かつ終夜睡眠ポリグラフ検査にて一晩の睡眠中（約7時間）に10秒以上続く無呼吸が1時間当たり5回以上出現する場合を睡眠時無呼吸症候群（SAS）と定義した。当時の日本では稀な疾患と考えられており，睡眠時無呼吸症候群の検査や診療を行う施設は数少なかった。しかし，Youngら^2,3）は，睡眠呼吸障害の有病率は成人男性の24％，女性の9％と報告し，中等度から重症の閉塞型睡眠時無呼吸症候群（OSAS）中，男性の82％，女性の93％が診断されていないことを報告した。わが国では，岡田^4）や大田ら^5）の調査より，日本人における睡眠時無呼吸症候群の有病率は，1～2％，1.3～2.7％とそれぞれ報告されている。

　睡眠時無呼吸症候群は，閉塞型，中枢型，チェーンストークス型，低換気型などに分類されているが^6），大半は上気道が狭窄，または閉塞した閉塞型である。OSASは，高血圧のリスクを増加させ^7），心血管疾患^8）や予後^9～11）を低下させることがわかってきた。ここでは，成人のOSASの病態や診断検査を中心に述べる。
