喘息と慢性鼻副鼻腔炎

Ⅰ　はじめに

　最近アレルギー性鼻炎や喘息を合併する慢性鼻副鼻腔炎が増え，以前わが国で多くみられた感染性慢性鼻副鼻腔炎（いわゆる蓄膿症）は少なくなった。喘息を合併している慢性鼻副鼻腔炎は，一般に好酸球浸潤を特徴とし，多くが嗅覚障害をきたし，手術しても再発しやすく，しばしば治療に難渋する。

　アレルギー性鼻炎と喘息は高率に合併し，1つの臓器として気道に起こる同じ炎症病態と考えられ，上・下気道の病態をまとめて把握しようとするone airway，one diseaseの概念が確立している。鼻粘膜と気管支粘膜だけでなく，副鼻腔粘膜も同じ起源の気道上皮であり，慢性鼻副鼻腔炎と喘息も同様な気道の炎症病態と考えられつつある。

　アレルギー性鼻炎と喘息はⅠ型アレルギー反応によるone airway，one diseaseであるが，慢性鼻副鼻腔炎に合併する喘息は，成人発症の喘息であることが圧倒的に多く，必ずしもⅠ型アレルギー反応による好酸球性炎症ではない。しかしアレルギー性鼻炎を含めたこれら気道炎症はいずれもTh2型炎症であり，Th2サイトカイン（IL-4，IL-5，IL-13など）とシスティニルロイコトリエン（CysLTs）の発現増強が気道粘膜だけでなく，骨髄や血中にもみられ，好酸球性炎症を引き起こしていると考えられる。これら2つの因子を抑制することが気道好酸球性炎症の治療には重要である。
