Ⅰ はじめに
メニエール病には難治性に移行する症例が存在する一方，自然治癒する症例もあり，難治性メニエール病に対して行う手術療法に有効性があるか否かは常に議論の的となってきた1,2）。手術療法の有効性を証明するためには，手術が必要であろうと判断される難治例にあえて手術をせずに経過観察する対照群を準備する必要があり，良質なevidence-based medicine（EBM）を得るのは困難な場合が多い。実際に難治性メニエール病に対する手術療法の論文は多いが，それらのほとんどは単なる治療成績の報告であり，対照比較試験を行っているものは少ない。難治性メニエール病の手術療法に関する良質なEBMが存在しないことは大きな問題であり，そのためにやむを得ず無効な保存療法を漫然と続けることになれば，高度感音難聴の進行3），さらには両側メニエール病への移行4,5）と，患者のQOLは著しく低下していくことになる。
難治性メニエール病に対して求められる手術療法は，少なくともめまい発作に対して効果を有し，術後さらなる耳鳴・難聴，平衡失調などの合併症が生じる可能性の低い安全なものが期待される。ここでは手術療法のうち，内耳機能温存さらに改善を意図して行う内リンパ囊手術，内耳機能のうち選択的に前庭機能の抑制，廃絶を意図して行うゲンタマイシン鼓室内投与および前庭神経切断術に関するめまい発作抑制，聴力保存・改善成績について述べる。
