成人スティル病を合併した下咽頭癌の放射線性喉頭壊死症例

Ⅰ．はじめに

　喉頭壊死は頭頸部悪性腫瘍に対する放射線治療により起こりうる稀な晩期合併症であり，照射終了3か月後から12か月後に起こることが多い^1）。重度の喉頭壊死に対してはしばしば外科的治療が選択されるが，照射後早期に発症し手術が不可能な状態に陥った報告例は少ない。今回，われわれは成人スティル病を合併しており長期にわたる副腎皮質ステロイドの投与がなされていた下咽頭癌の症例で，放射線治療後早期に喉頭壊死をきたして死亡した1例を経験したので，若干の文献的考察を加えて報告する。
