Ⅰ　HPV感染症―特に喉頭気管乳頭腫症について

　ヒトパピローマウイルス（human papilloma virus：HPV）は，2本鎖DNAウイルスで，その感染に際しては，種特異性が高い。HPVの遺伝子情報は，非構造蛋白質をコードする初期遺伝子群と，構造蛋白質をコードする後期遺伝子群および遺伝子をコードしていない発現調節領域から構成されている。

　HPVは皮膚・粘膜の表皮の小さな傷を通して表皮最下層の基底層に感染し，基底層とその上の有棘層に潜伏する。細胞分化とともにウイルスゲノムは複製され，顆粒層から角質層といった，表皮の上層にかけてウイルス粒子が形成される。この位置関係から，抗原性のあるウイルス粒子は血中に侵入せず，また基底膜下の抗原呈示細胞に曝露されないことから，免疫応答による排除を受けにくい。
