Ⅰ　疾患についての概説

　線維素性唾液管炎とは，1879年Kussmaul^1）によって初めて報告された発作性反復性に唾液腺腫脹をきたす疾患である。末しょう腺組織ではなく主に導管系が閉塞し，唾液管開口部からは白色の索状分泌物が排出されるのが特徴的である。この線維素塊の中には多数の好酸球が認められる。唾液腺造影では，主導管の高度な拡張像を呈する。これまで国内外からの報告は約40例と決して多くはないが，Pearson^2）は耳下腺の反復性腫脹を伴う患者104名中16例（15.4％）に本疾患を認めたと報告している。このように本疾患に対する認識の低さから臨床上，見逃されている可能性もあると考えられる。そこで反復する唾液腺腫脹を主訴とする場合には，本疾患を念頭に入れておくことが重要であると思われる。
