耳下腺に発症したメトトレキサート関連リンパ増殖性疾患の2例

はじめに

　メトトレキサート（methotrexate：MTX）は，葉酸に拮抗することでDNA合成/細胞複製を阻害する作用があることから，関節リウマチ（rheumatoid arthritis：RA）をはじめとした膠原病^1），乾癬（重症例），移植後反応，中枢神経領域のリンパ腫など特定のリンパ球系細胞腫瘍の治療にも用いられている。近年，MTXを内服している患者にリンパ増殖性病変が生じうることが明らかとなりMTXが広く用いられるようになるにつれ同様の報告例が増えてきた。

　MTX関連リンパ増殖性疾患（methotrexate-associated lymphoproliferative disorders：MTX-LPD）は，MTXによる免疫抑制状態の患者に生じたリンパ増殖性病変または悪性リンパ腫と定義され，新WHO分類（WHO-2001）には1998年の試案の段階で取り上げられ，免疫不全関連LPD（immunodeficiency-associated LPD：ID-LPD）のなかで，一次性免疫異常症関連LPD，human immunodeficiency virus（HIV）関連リンパ腫および移植後LPDに次いで第4の独立項目とされた^2）。

　今回われわれは耳下部腫脹を主訴に受診した耳下腺に発生したMTX-LPDの2例を経験したので報告する。
