咽頭癌に対する機能温存手術—経口的切除術

POINT

●経口的切除術は低侵襲，機能温存治療の1つとして，近年機器の発達とともに普及しつつある。

●さまざまな術式があり，世界的にはレーザーを用いたTLM，手術用ロボットを用いたTORS，わが国では内視鏡下手術TOVSなどが開発されてきた。

●経口的切除術の治療成績は良好であり，適切な症例の選択を行えば低侵襲な機能温存手術として有用性が高い。

●化学放射線治療後の救済手術や，導入化学療法による腫瘍縮小症例に対する機能温存手術としても，適応可能であれば効果的な治療となりえる。

●N0症例ではセンチネルリンパ節生検と組み合わせることで，原発巣に対する低侵襲手術と頸部郭清術省略が可能となり，低侵襲・個別化治療として期待される。
