喉頭浮腫を生じた遺伝性血管性浮腫の1例

はじめに

　発作性に局所的な浮腫を繰り返す疾患は，「クインケ浮腫」^1）と呼ばれ，原因不明であるが予後良好な疾患としてそれほど重要視されてこなかった。しかしクインケ浮腫のなかには気道閉塞に至る危険性のある重篤な疾患も含まれている。遺伝性血管性浮腫はその1つであり，C1インヒビター活性の低下により，ブラジキニンや補体分解産物が過剰産生され，血管透過性が亢進することによって発作性・限局性浮腫をきたす疾患である。

　遺伝性血管性浮腫による喉頭浮腫に対しては，一般的に抗ヒスタミン薬や副腎皮質ステロイド薬は必ずしも有効ではなく，気道確保，C1インヒビター製剤投与などの適切な治療を行わなければ30〜56％の患者が死に至る^2〜5）。今回われわれは，遺伝性血管性浮腫による喉頭浮腫の1例を経験したので，文献的考察を加えて報告する。
