異所性過誤腫性胸腺腫の1例

はじめに

　異所性過誤腫性胸腺腫（ectopic hamartomatous thymoma：EHT）は1982年にSmithら^1）が報告した疾患概念に始まり，1984年にRosaiら^2）によって名づけられた非常に稀な病変である。臨床的には鎖骨上部，胸骨上部・前部などに発生し，弾性軟で皮膚との癒着がない良性腫瘍の特徴をもつ。画像検査で周囲組織との境界は明瞭もしくは不明瞭で，内部不均一に描出されることがあり，病理診断からも滑膜肉腫や悪性神経鞘腫などとの鑑別がしばしば問題となる。確定診断を得るには本疾患を念頭に置いて精査にあたる必要がある。

　今回，われわれは胸骨上部に発生したEHTの1例を経験したので，若干の文献的考察を加えて報告する。
