悪性外耳道炎の診断と治療

POINT

●悪性外耳道炎は重症化すると頭蓋底骨髄炎に進展して，致死的になりうる疾患であり，早期に診断して抗菌薬の長期投与を行うことが肝要である。

●適切な外耳炎の治療を継続しているにもかかわらず病状が悪化する場合には，悪性外耳道炎を疑いCTおよび造影MRI検査を行うことが早期診断のカギとなる。

●悪性外耳道炎は特異的な検査結果が少なく，確定診断のためには病理組織学的検査でほかの疾患を除外することが不可欠である。

●起炎菌は緑膿菌の頻度が高いが，MRSAや真菌による悪性外耳道炎の報告が増加しており，起炎菌の同定および感受性検査が重要である。

●頭蓋底に炎症が波及した場合には，少なくとも6週間以上の抗菌薬投与が必要である。

●既存の治療法を行っても増悪の一途をたどる悪性外耳道炎の場合，抗菌薬動注が有効な可能性がある。
