頸部郭清術後の肩関節障害の予防とリハビリテーションの検討

はじめに

　頸部郭清術は頭頸部癌の治療にはかかせない手術手技であり，その必要性は広く認知されている。郭清範囲や術式（図1）は多少異なるものの，頸部を広く開創し，脂肪組織に含まれたリンパ節を一塊として摘出することは同じである。多少の後遺症が出現するのはやむを得ないとはいえ，患者にしてみれば少しでも後遺症が少ない状態を望むのは当然である。以前は転移性頸部リンパ節があれば根治的頸部郭清術が必須であったが，最近では内頸静脈と離れているものは，内頸静脈を切除せず保存することが普通になっている。副神経や胸鎖乳突筋に対しても同様で，リンパ節が神経や筋肉に癒着したり，播種が疑われる場合のほかは温存されている。最近は癌治療後のQOLの維持を重視するようになっている。

　頸部郭清術による肩の運動制限や肩の痛みは決して軽いものではなく，術後長期間継続するために患者のQOLは著しく低下する。頸部郭清術の合併症を予防するためには，郭清範囲の縮小（特にⅡB領域）やリハビリテーションの早期導入が考えられている^1）。
