放射線治療後に紡錘形細胞癌として再発した声門扁平上皮癌症例

はじめに

　紡錘形細胞癌（spindle cell carcinoma：SpCC）は，上皮内または浸潤性扁平上皮癌（squamous cell carcinoma：SCC）と，上皮性であるが間葉系腫瘍の様相を呈する紡錘形腫瘍細胞の2相性細胞で構成された特異な腫瘍である^1）。

　喉頭におけるde novo発生は喉頭悪性腫瘍の1.7〜2.7％と報告されているが^1,3），放射線治療したSCCが再発したあとSpCCと診断された報告は稀である^1）。

　今回われわれは，左声門SCCを一次治療したあと，気管，声門下と2度の再発・転移を繰り返し，最終的にSpCCの病理組織学的診断となったが制御された1症例について，若干の考察を加えここに報告する。
