成人に発生した鼻副鼻腔原発横紋筋肉腫の1例

はじめに

　横紋筋肉腫は骨格筋へ分化する間葉系細胞から発生すると考えられている稀な軟部肉腫であり，アメリカでは年間250例が新規に診断されている^1）。主に小児期に発症するが，実際は10人中4人が成人例で人口100万人に対し年間4.3人の成人罹患率があるとされる^2）。近年，化学療法により予後の改善が認められるようになりつつあるが，進行例の予後は依然としてきわめて不良で，high risk例の治療奏効維持3年生存率（failure free survival）は30％未満である^3）。今回われわれは鼻副鼻腔に発生した成人の横紋筋肉腫の進行例に対し集学的治療を行った例を経験したので報告する。
