外リンパ瘻の診断基準

POINT

●外リンパ瘻は，内耳リンパ腔と周辺臓器のあいだに瘻孔が生じ，生理機能が障害されてめまい，耳鳴，難聴などが生じる疾患である。瘻孔から外リンパが漏出すると，症状が増悪，変動する。

●新しい診断基準では「瘻孔が確認できたもの，もしくは外リンパ特異的蛋白が検出されたもの」となった。

●外リンパ瘻の診断基準を発表しているのは日本のみで，諸外国からの発表はない。診断・治療方針は国によって大きく異なる。

●外リンパ瘻は発症の誘因によりカテゴリー1から4まで分類された。これらを別々に論じることが大切である。

●CTP検査は侵襲が少なく客観的な評価指標であり，外リンパ瘻の診断に有用である。
