POINT

●口腔がん・中咽頭がんともに，郭清範囲は，原発巣の大きさや浸潤の程度，リンパ節転移の有無により決定される。

●通常，舌骨近傍領域は郭清範囲外だが，同部位に転移が認められ頸部郭清を実施する場合には，同領域も治療域に含めることが望まれる。

●臨床的にN0と診断された中咽頭がんでも，潜在的なリンパ節転移が疑われる場合には，予防的郭清を行う。

●HPV陽性中咽頭がんであっても，頸部郭清の範囲を縮小することは推奨されない。

●口腔がん，特に早期舌がんN0症例に対する予防的郭清については，さまざまな意見があり結論は出ていないが，センチネルリンパ節生検による手術適応の可否決定の有用性が報告されている。
