肩甲舌骨筋上頸部郭清術

POINT

●肩甲舌骨筋上頸部郭清術は，選択的頸部郭清術の一種であり，レベルⅠ〜Ⅲに含まれるリンパ節を一塊に切除する。

●主に口腔がんN0症例の予防的郭清，とりわけpull-through法による原発巣切除が必要となる症例や，遊離組織移植例に対して術野と再建血管の確保を兼ねて適応される。今後の臨床研究の結果によって，舌部分切除術単独例にも拡大されるかもしれない。

●当該領域の過不足ない切除を心がける。とりわけ郭清の下端となる肩甲舌骨筋上腹が，レベルⅢの下端の高さと必ずしも一致しない点に注意を要する。
