POINT

●耳鼻咽喉科医が主に扱うのはIgG4関連唾液腺病変（ミクリッツ病とキュットナー腫瘍）である。

●診断は，症状，高IgG4血症，リンパ球・IgG4陽性細胞浸潤と線維化の組織所見による。

●典型的ミクリッツ病は臓器別診断基準を，キュットナー腫瘍は包括診断基準も用いる。

●確診例であっても，唾液腺単独病変，治療不要・寛解例は重症に分類されない。

●重症は，ステロイド抵抗性・依存性，他臓器障害（腎臓，膵臓など）のある症例である。

●ステロイド抵抗性・依存性は治療開始後6か月で判定する。

●生検が難しい臓器病変の診断確定のため，唾液腺病変の診断・生検が求められることがある。
