POINT

●良性発作性頭位めまい症（BPPV）の歴史を知ったうえでの診断，治療が必要である。

●「BPPV」という用語の定義があいまいになり，乱用される傾向にある。このため，広義と狭義のBPPVに分けるなど用語の整理が必要である。

●多くの医療者はBPPVの治療はレシピのように順番に行っているが，眼振を観察しながら臨機応変に治療法を変える必要がある。

●BPPVの再発を論じる前に，前の治療で完治したかを検討する必要がある。また，内耳障害などの合併を伴うBPPVは再発しやすいことが知られ，このほかに併発する疾患としての認識も必要である。

●浮遊耳石置換法（CRP）を考案したEpleyは開業医であるが，定説に反した医療を行ったのは，「自身が診たことのほうが正しいと確信していたから」と述べている。この精神は現在世界的に高い評価を得ている。
