扁桃と睡眠時無呼吸症候群

POINT

●小児の閉塞性睡眠時無呼吸（obstructive sleep apnea：OSA）は，睡眠・呼吸の障害が健康な成長発達を妨げる疾患である。

●扁桃肥大・アデノイドはOSAの主な原因であるが，重症度を示すものではない。

●OSAの重症度診断の基準は確定されたものはなく，全身の状態，OSA-18問診票や動画撮影，多点感圧センサーなど多角的な評価が重要である。現状では携帯型モニターの信頼性は低いので，注意が必要である。

●扁桃肥大・アデノイドを認める症例では，扁桃摘出・アデノイド切除が第一選択となる。保存的に経過をみる場合でも，半年程度で重症度の再評価が必要である。

●扁桃肥大・アデノイドを認めない重症例，合併症をもつ症例，36か月未満の乳幼児例は専門機関への紹介が望ましい。
