彎曲型喉頭鏡により原発巣を同定し得た頸部リンパ節転移癌の1例

はじめに

　癌診療の現場では，細胞学的に癌が証明された頸部リンパ節転移を認めても原発巣を見出せず，原発不明頸癌と診断される症例がある。頭頸部外科領域において，頸部リンパ節腫脹を初発症状とする原発不明頸部リンパ節転移癌は全頭頸部癌の1〜5％を占めると報告されている^1-3）。治療は，患側の頸部郭清術と口蓋扁桃摘出術を施行し，術後の病理組織診断結果に応じて化学放射線療法を追加する施設が多い。

　より適切かつ効果的な治療法を選択するためには，原発巣の確定に努めることが非常に重要である。しかし，残念ながら診断技術の進歩が目覚しい今日においても原発巣判明率を向上させることは容易ではなく，診断や治療に苦慮することが多い。

　今回われわれは全身麻酔下において彎曲型喉頭鏡による喉頭展開を行い，下咽頭梨状陥凹ならびに食道入口部に存在する粘膜病変（原発巣）を確認し，的確な根治治療を行い得たリンパ節転移癌の1例を経験したので報告する。
