頭頸部扁平上皮癌患者における栄養評価指標と予後の関連

はじめに

　頭頸部扁平上皮癌患者は，高齢発症，併存症，腫瘍による嚥下障害のため，低栄養状態にあることが多い^1）。低栄養は治療中断や合併症の増加と関連するため，適切に栄養状態を評価し，栄養療法を施行する必要がある^2）。栄養状態を評価する指標としては，body mass index（BMI），末梢血中リンパ球数，血清アルブミン濃度，血中ヘモグロビン濃度が用いられることが多い^3,4）。これらの栄養指標は，頭頸部扁平上皮癌患者の予後と相関することが知られている^5,6）。しかし，これらのうち，どの項目が最もよく予後と相関するかについては，いまだ明らかにされていない。今回われわれは，根治的放射線療法を受けた頭頸部扁平上皮癌患者を対象に，栄養評価指標と生命予後との関係を検討したので報告する。
